Every Dollar Matters: Reduce Your 340B Risk and Drive Pharmacy Revenue in 2027

By Adam Rosenberg

With the 340B Rebate Pilot and newly finalized OPPS rule colliding to put pharmacy finances at risk, teams must look for ways to lower drug costs, drive revenue growth, and protect rebate dollars – all while staying compliant.

Most organizations are not prepared to tackle even one of these challenges, let alone all of them at once. But there is no other option: the problems converge in January 2027.

Converging problems for pharmacy and finance teams

The rebate model puts pharmacy dollars at risk

The 340B rebate pilot (planned for January 1, 2027) converts the upfront 340B discount to a retrospective rebate on applicable drugs. Combined with MFP refunds, this means health systems will pay the full WAC price upfront and wait up to 55 days to recover what they’re owed. Based on Bluesight data, the average health system will have $760k in cash tied up in 340B rebates next year, and that number only grows in 2028 if the Pilot expands or moves out of a pilot phase.

The model also creates operational jobs nobody owns today: chase every rebate before its window closes, prevent duplicate claims across two programs, and maintain a defensible record for every dollar claimed so you can respond to denials and clawbacks, and file GFIs. HRSA estimates the rebate model will add more than $34,000 per facility per year in administrative burden alone. Many organizations estimate their costs could be in the millions.

Margin compression is arriving faster than most teams can adapt

On November 4th, CMS finalized a rule cutting Medicare Part B reimbursement for 340B-acquired outpatient drugs, creating even more need to protect reimbursements and identify revenue-driving opportunities throughout the pharmacy enterprise.

As care continues shifting into outpatient settings, pharmacy is increasingly tasked with margin protection and revenue growth at the exact moment revenue recovery is hardest to achieve. Unfortunately, pharmacy and revenue cycle teams work from their own data, so the net dollar result of any purchase decision stays invisible until it’s too late to act.

340B compliance can’t scale with manual processes

One thing that remains essential in 2027 is maintaining a compliant 340B program that ensures ongoing access to lower drug prices through up-front discounts or rebates. In fact, the rebate pilot should be viewed as much a harbinger of tighter compliance and rigorous oversight as a shift in the financial model itself. Sample-based self-audits typically cover less than 5% of 340B transactions, leaving most of a covered entity’s program without meaningful oversight. As organizations expand services, add contract pharmacies, and see transaction volumes rise, the likelihood of missed errors, compliance risk, and unrealized savings increases.

HRSA requires transaction-level records across multiple sources to demonstrate compliance and maintain OPAIS accuracy. That requirement only gets harder to meet as the rebate model adds new layers of documentation to the equation, and additional legislation currently under review would add tighter oversight on top of it.

Navigating changes with Bluesight

In 2027, hospital pharmacies must build processes and leverage technology to:

  1. Stay compliant to protect your 340B program and maintain access to 340B prices
  2. Identify every rebate-eligible claim and submit it efficiently
  3. Track rebate dollars to effectively monitor cash flow
  4. Drive reimbursement through revenue-aware drug selection and remittance oversight

To support organizations with compliance, rebate tracking, and revenue generation, Bluesight has designed an end-to-end solution:

  • To ensure 100% compliance, 340BCheck audits every 340B transaction, syncs with OPAIS daily, and keeps all program documentation current so issues are resolved before HRSA even arrives. When the rebate pilot goes live, the solution will identify and confirm eligibility before submitting each claim, track submission and payment status, prevent duplicate claims across the 340B and MFP programs, and maintain the record you need to respond to any denial or clawback.
  • To reduce costs, drive revenue, and track rebate dollars, CostCheck provides purchasing decision support to identify lower-cost drug options in DRG-based reimbursement care areas and identify revenue expansion opportunities, based on actual reimbursement rates. The solution also identifies overcharges, catches contract errors, and provides preferred purchasing oversight. As a registered TPSE connected to Beacon, our solution ties rebates back to the original invoice, showing what’s been paid, what’s outstanding, and the true net cost for every drug.

Bluesight helps organizations protect top-line revenue, collect every rebate dollar, and manage cash flow amid historic OPPS reimbursement cuts and a new paradigm for 340B discounts.

The proof is already visible: health systems using CostCheck save upwards of 9% on annual drug spend, with $3M or more in new revenue opportunity identified. And zero 340BCheck customers audited by HRSA in FY25 received adverse findings.
